To become a Western Iowa Telephone Cooperative member, we’ll need the following forms either FAXed, mailed, or dropped off at our business office:

• Signed Membership Form
• Completed Service Application
• Completed Long Distance PIC Change Form
• PIC Freeze Form (if applicable)
• Additional Calling Features Form (if applicable)
• Internet Application (if applicable)
• Cable TV Application (if applicable)
